Do you like seeing your kids in Video and Photos in Brightwheel? Wanna see them on our Facebook Page as well? In order to have that happen you will need to submit a form giving us permission to use your child's likeness in videos and photographs in areas you indicate below (ie., Brightwheel, the La Casita Facebook Page, and Our Website).
Adding your name in Parent Name field being submitted serves as your signature on this permission form. *You must submit one form for each child in our care. If you wish to revoke permission given, simply resubmit the form with any changes.

May we use photo and video of your child to market La Casita in brochures or promo video?
